PCA has developed Member Safety Guidelines for Club Events. Please review by clicking this link: PCA Safety Guidelines.
Print the downloaded form and fill in your data , sign it and scan it or take a high-res picture.
Each member and each co-member needs to provide a signed release.
The release is valid until February 28, 2022 and will cover all club events during that time frame.
Fill in the fields in the form below and click “Choose File” and then select your scanned file or picture.
If you are a member and logged in, most of the fields in that form will be pre-loaded with your data.
